Fill the details and submit.
You will need to provide photos of the front and back of the patient and person who will pick up the prescription.
You will also need to send a photo of your prescription. If you have a blue prescription for controlled medication, upload a photo of both prescriptions together (sometimes they accept just a photo of the blue prescription as well).
